We have three locations for your convenience:

• Milford Hospital

Initial Visit

On your initial visit, please bring the following items:

• Identification
• Insurance card(s)
• Written referral form, if required
• List of medications you are taking
• Pertinent test results

Also, be prepared to share your medical history with our physicians. This will include any medical conditions you have or may have had in the past, surgical history, any medications (prescription or over-the-counter) you are currently taking (include name, dosage and frequency), and any allergies you have to medications or foods.

Insurance

We accept most major insurances, including Blue Cross of Michigan, Medicare and Medicaid. Please be sure to bring your insurance card, referral if necessary, and identification to your office visit. Please arrive early to your first appointment to allow time to complete your medical history and demographic forms.
